Five Patient Case Study

Case Study - Allison P. Twenty-five year-old female, blind, deaf and paralyzed from tumors of Neurofibromatosis 2, had several deep pressure sores covering nearly all of her left buttock for 14 months. Wounds were further inflamed and compromised by constant urinary incontinence. Due to location and discomfort of tumors, patient continues lying on open wounds which continued to worsen. Case Study - Richard T. Fifty-seven year old male residing in nursing home due to complications of diabetic neuropathy. Had chronic diabetic ulcer on bottom of right foot, 2 inches behind 4th and 5th toes. Wound measured 1 half inch wide and was surrounded by thick callous the size of a 50 cent piece. Sixty-eight year old male paraplegic, spinal cord injured T-3, post 38 years, had been hospitalized in an attempt to get a serious decubitus ulcer on his tailbone small enough to do a flap. After 11 months of inpatient wound care, wound was still 3.9 cm deep and 3.5 to 4 cm across. Skin integrity was compromised and inflamed for 6 - 8 inches on buttocks surrounding the wound. Case Study - John T. Fifty year old male, spinal cord injured, T-4, post 20 years. Patient began getting sores on buttocks and heel within 12 mo of onset. Some wounds have continued for 19 yrs, not responding to traditional treatment. Eventually wounds covered all of his buttocks and large portions of his upper thighs. Patient was hospitalized frequently for wound infections,including MRSA. Patient had several unsuccessful surgeries to close wounds. Case Study - Sharon G. Fifty-eight year old female, spinal cord injured, T-9, post 44 years, had history of periodic pressure sores on back of left heel. She presented to Physical Therapy in June of 2004 with wound measuring 1.2 x 1.7 cm. Out patient treatment continued three times a week for 16 weeks, including debriding when needed. Various dressings were tried. Some of them were initially effective for a few days before the wound would regress again with undermining, tunneling and maceration. One incident of severe infection required IM injections of Rocephin for 10 days. Wound grew to 4 x 6 cm and emitted odor despite meticulous care.
